Potential new patients must complete a pre-screening questionnaire prior to acceptance and scheduling an initial appointment.  This step is essential to ensure Dr. Hall is the right provider for you.

All visits can be completed in person and/or via telehealth based on your preference.  Please be advised that telehealth appointments are only available to individuals located in Vermont.
